What is adhd?

ADHD is usually described as having two dimensions. Inattention covers difficulty sustaining focus, losing things, missing details, being easily diverted and struggling to finish what was started. Hyperactivity and impulsivity cover restlessness, difficulty remaining seated, interrupting, and acting before thinking it through. A person may have mostly one, mostly the other, or both.

Two things routinely delay diagnosis. Children who are inattentive but not disruptive tend to be described as dreamy rather than referred, and girls in particular are missed for this reason. And in adults, the visible hyperactivity often fades while the inattention, disorganisation and impulsivity remain — so the person is assessed for anxiety or depression instead, which may also be present, without the underlying ADHD ever being considered.

  • Work or study is suffering despite genuine effort
  • Relationships are strained by forgetfulness, lateness or impulsivity

Assessment across settings

ADHD is diagnosed from a pattern present in more than one setting and beginning in childhood. That means a developmental history, information from family and, for children, from school. Rating scales support this; they do not replace it.

Ruling out what looks similar

Poor sleep, hearing difficulty, learning disorder, anxiety, depression, thyroid problems and substance use can all produce inattention. Each is checked, because treating the wrong one wastes time.

Practical and behavioural strategies

Structure, externalised reminders, broken-down tasks, adjustments at school or work, and consistent routines. For children, work with parents on behaviour management is a core part of treatment.

Medication where appropriate

Medication is well established in ADHD, and is also not automatic. Whether it is suitable, what is expected of it and how it is reviewed are discussed in the consultation. No medicine is named or recommended here.
